Homeleigh Nursing Services provides home healthcare solutions and specialized nursing care and in order for our multi-disciplinary team to implement any services, we require your informed consent to do a nursing assessment, either physically, virtually, or telephonically.
Where it is required or requested by the client to have a representative to act on the client’s behalf, we require such informed consent from the representative.
It is then hereby agreed that the client/representative, having been appointed by the client to act as representative / the legal guardian of the child (child as determined by the Children’s Act, No. 38 of 2005) or competent person acting on behalf of the child, that a medical assessment may be performed which may include any of the following:
The client/representative further agrees and consents to the processing of any personal information deemed necessary and applicable by the Homeleigh multi-disciplinary team to enable them to recommend a service specific to the client’s needs.
Such personal information may include medical history, health and gender, biometric information and medical information held by a third party. The client further consents to such personal information being shared within the Homeleigh functional departments as is needed to facilitate the implementation of the service.
It is further understood that the client’s personal information may be shared with third parties such as medical aids, and/or any external medical professionals, and hereby consent to this with the understanding that the client may at any time request the details of such sharing of information with a third party.
The client confirms that he/she has disclosed all relevant medical information known to him/her, and to his/her knowledge, it is true and correct. As per the Protection of Personal Information Act, Act 4 of 2013, and using the prescribed procedure, the client understands that he/she may at any time request access to the client’s personal information to amend or delete it.
Homeleigh will take appropriate and reasonable security measures to safeguard the integrity and confidentiality of personal information and will not share any information concerning our client other than with the appointed representative if one has been appointed.
